D4W Marketing Post Visit Survey Link

The following steps will ensure the Post Visit Survey template uses the correct electronic form URL when sending to patients. If this is not completed, patients will receive the e-mail with no link, defeating the purpose of the communication. 

1. Copy the Feedback/Survey Form URL, if using eForms see instructions

2. Go to Location Setup > General tab

3. Group: eMarketing 

   1. Post Visit Survey Link: Paste the form URL
